A new concept analysis published in Cureus warns that the frontline workers most essential to patient care are being left out of the design of healthcare artificial intelligence. The study introduces the construct of the "invisible human-in-the-loop" to describe how direct-care workers are critical to the functioning of AI systems yet remain overlooked in their development.

The research focuses specifically on Certified Nursing Assistants (CNAs), who represent the largest group of direct-care workers in the United States. According to the study, these professionals perform up to 90% of direct-care tasks—including feeding, dressing, bathing, and resident movement—and number over 500,000. Despite this massive footprint, the authors argue that the relationship between these workers and the technology they use is rarely clarified in health informatics agendas.

The Gap in Health Informatics

CNAs provide the vast majority of physical care in nursing home settings and are the primary source of the raw data used in resident care planning. As AI is integrated into these environments to automate monitoring or predict patient needs, there is a growing risk that the workers who actually implement and validate these systems on the ground remain invisible to the engineers and administrators designing them.

Failure to include the direct-care workforce in the design process can lead to a disconnect between theoretical AI capabilities and the practical realities of bedside care.
