The University of Rochester has launched a pilot of an AI-powered virtual patient platform designed to help dental residents refine their communication skills. The tool, known as SOPHIE (Standardized Online Patient for Healthcare Interaction Education), allows clinicians to practice navigating patient anxiety and difficult conversations before entering a real clinical setting.
Developed through a multidisciplinary collaboration between the Eastman Institute for Oral Health (EIOH), the University's Department of Computer Science, and the Clinical and Translational Science Institute, SOPHIE utilizes a 3D virtual character. This AI entity is capable of seeing, listening to, and responding to clinicians in real time via a standard web browser. The training curriculum is specifically structured around the "three E's": Empower, Empathy, and Explicit communication. The results of the pilot program were published in the Journal of Dental Education.
The Shift from Human Actors
Traditionally, healthcare professions have relied on "standardized patients"—trained human actors—to teach the nuances of bedside manner and communication. While effective, the human-led model is resource-intensive and limits the frequency with which a student can practice a specific scenario. SOPHIE was originally engineered for physicians managing patients with serious illnesses, but was later adapted for the dental field to specifically address common patient fears and misconceptions.
Addressing Dental Anxiety
Dental anxiety remains a significant barrier to patient care, often preventing individuals from seeking necessary treatment. By providing a scalable, low-pressure environment, the technology allows dentists to master active listening and empathy without the risk of a failed first encounter with a live patient. As Jin Xiao, DDS, PhD, an EIOH professor, noted, "Great dentistry is not only about fixing teeth, but also caring for our patients."
The Role of AI in Empathy
While the platform simulates human interaction, the developers emphasize that the technology is a supplement to, rather than a replacement for, human connection. Ehsan Hoque, PhD, a professor of computer science, stated that while AI cannot replace empathy, it provides a space where clinicians can repeatedly practice the skills patients depend on without the pressure of a real clinical encounter.
