Johnson & Johnson MedTech secured FDA De Novo authorization on July 22, 2026, for its OTTAVA Robotic Surgical System—the first table-integrated soft-tissue robotic platform to enter the U.S. market. The clearance challenges Intuitive Surgical's da Vinci system, which has commanded approximately 80% of the soft-tissue robotic surgery market for two decades.
A Robot Built Into the Table
Unlike conventional robotic systems that rely on floor-mounted carts or overhead booms, Ottava integrates four robotic arms directly into a standard-sized surgical table. The arms stow flush when not in use, occupying 30-50% less space than traditional boom-and-cart configurations. "The table is a robot and the robot is a table," said Rocco De Bernardis, J&J's global president for Ottava.
The design addresses a persistent constraint in operating rooms: crowded physical layouts that limit robotic adoption in facilities not originally equipped for large cart-based systems. Ten Procedures Cleared
The FDA authorization covers ten general surgery procedures: Roux-en-Y gastric bypass, gastrectomy, cholecystectomy, splenectomy, gastric sleeve, small bowel resection, appendectomy, lysis of adhesions, fundoplication, and hiatal hernia repair. The De Novo pathway establishes a new device classification for table-integrated robotics, a regulatory precedent that could shape future entrants.
Twin Motion and Instrument Control
Ottava features synchronized "twin motion" capability, enabling coordinated movement of the table and robotic arms to reposition patients mid-procedure without interrupting the operation. The system also deploys a two-in-one needle driver with surgeon-activated "cut only" and "suture only" modes, designed to reduce unintended suture cutting during procedures.
Market Implications
While competitors like Medtronic's Hugo have entered the soft-tissue robotic space, most have followed the cart-based architecture pioneered by Intuitive. Ottava's table-integrated approach represents a structural departure that could reshape operating room workflows and expand access to robotic-assisted surgery in space-constrained facilities. The system's commercial rollout timeline and pricing have not been disclosed.
