Supercentenarians possess 'hypervigilant' immune cells that ward off cancer

Research from the University of Osaka reveals that people aged 110 and older exhibit a unique expansion of specialized T-cells that fight infection and malignancy.

Extreme longevity may be driven by an immune system that adapts rather than decays. Research led by Kousuke Hashimoto at the University of Osaka suggests that supercentenarians—individuals who have lived beyond 110 years—possess a "hypervigilant" immune state that provides a critical defense against cancer and infections in extreme old age.

The study found that supercentenarians have a disproportionately high percentage of CD4 cytotoxic T lymphocytes (CD4 CTLs). These are specialized immune cells capable of identifying and destroying both infected cells and cancerous cells. Data indicates that the proportion of these cells relative to total T-cells increases sharply with age: they make up approximately 4% in individuals aged 70-99, nearly 10% in those aged 100-109, and jump to over 17% in supercentenarians.

Challenging the decline of aging

Traditionally, medical science has viewed the aging process through the lens of "immunosenescence," the theory that the immune system undergoes a gradual and inevitable decline in function. However, the University of Osaka team argues that the biological reality for the world's oldest people is different. Hashimoto noted that rather than simply declining, the immune systems of these exceptionally old individuals may retain the ability to adapt to the persistent challenges that accumulate over a century of life.

This finding builds on previous work by the researchers, who identified the expansion of these cytotoxic CD4 T cells as a distinctive marker of the supercentenarian group in a study published in the Proceedings of the National Academy of Sciences (PNAS).

Implications for elderly care

If the expansion of CD4 CTLs is a primary driver of longevity and cancer resistance, it opens a new door for geriatric medicine. Understanding the specific biological mechanism that triggers this expansion could allow scientists to develop medical interventions that reproduce this beneficial immune response in the general elderly population. Such a breakthrough could potentially reduce cancer rates and improve survival outcomes for millions of older adults who do not naturally possess this genetic or adaptive advantage.

Future directions

While the correlation between CD4 CTL expansion and extreme longevity is clear, researchers continue to investigate the precise triggers of this adaptation. Further study is needed to determine if this hypervigilant state is a prerequisite for reaching age 110 or a result of the body's response to surviving into that age bracket. For now, the discovery suggests that the ceiling for human immune resilience is significantly higher than previously believed.
