A randomized clinical trial published in JAMA Network Open has found that immersive virtual reality (VR) tours of operating theaters significantly reduce preoperative anxiety in adults undergoing elective surgery. The findings suggest that providing patients with a digital walkthrough of their surgical environment can serve as an effective non-pharmacological alternative to traditional anti-anxiety treatments.

The study, conducted at The Chinese University of Hong Kong and registered under ChiCTR2100051690, involved 74 adult patients. Participants in the intervention group were given an eight-minute VR tour designed to familiarize them with the operating room. This immersive education aimed to reduce the fear of the unknown by allowing patients to visually experience the surgical setting before their procedure.

The Burden of Preoperative Stress

Preoperative anxiety is a widespread issue in clinical settings and is frequently linked to adverse physiological outcomes. High levels of stress before surgery can lead to elevated blood pressure, increased heart rates, and a higher risk of bleeding during the procedure. To manage these risks, hospitals traditionally rely on pharmaceutical interventions, such as benzodiazepines like midazolam. While effective at reducing anxiety, these medications often cause sedation and can prolong a patient's recovery time after surgery.

Implications for Patient Care

This research indicates that low-cost, consumer-grade VR hardware can provide a clinically significant reduction in patient stress without the side effects or costs associated with drug-based interventions. By shifting the focus toward patient education and environmental familiarity, healthcare providers may be able to improve overall patient preparedness and safety. The ability to lower anxiety without inducing sedation could potentially streamline the perioperative process and improve the immediate post-surgical experience for the patient.

Future Outlook

While the study serves as a successful proof-of-concept, researchers note that the sample size was relatively small. Further large-scale trials are needed to determine how these VR interventions scale across different types of surgeries and diverse patient populations. The next step for the industry will be determining whether integrating immersive education into standard perioperative care can lead to a measurable decrease in the administration of preoperative sedatives across broader hospital systems.
