Metaverse Training Helps Claustrophobic Patients Complete Cardiac MRIs

A feasibility trial shows virtual hospital simulations can enable heart disease patients to undergo critical diagnostic scans they previously refused.

Virtual reality is showing potential as a tool to help heart disease patients overcome the claustrophobia that often prevents them from receiving critical diagnostic imaging. A 2024 case series published in PMC found that patients who had previously refused cardiac magnetic resonance imaging (CMR) were able to successfully complete their scans after training in a metaverse-based virtual hospital.

In the preliminary feasibility trial, three patients with cardiomyopathy—all of whom had previously declined CMR due to claustrophobia—underwent VR training to simulate the experience. Following this virtual preparation, all three patients successfully completed their actual cardiac MRI scans. This intervention addresses a significant clinical hurdle, as claustrophobia affects approximately 10% of patients referred for these specific scans, according to the PMC report.

The Challenge of Cardiac Imaging

Cardiac MRI is a vital diagnostic tool for identifying and managing heart disease, providing detailed images of the heart's structure and function. However, the physical environment of the scanner—a narrow, enclosed tube—can trigger intense anxiety and panic in a significant minority of the population. When patients cannot tolerate the enclosure, critical tests are canceled, potentially delaying diagnosis or altering the course of treatment for serious cardiac conditions.

To combat this, researchers are testing various VR interventions. These range from brief virtual tours and procedural education to more structured, multi-session Virtual Reality Exposure Therapy (VRET). The goal is to desensitize patients to the environment and reduce distress before they ever enter the clinic.

Clinical Implications and Limitations

If VR can reliably overcome claustrophobia, it would prevent the cancellation of essential diagnostic tests for heart disease patients, ensuring that high-risk individuals receive the imaging necessary for their care. By simulating the sensory experience of the MRI suite, clinicians hope to bridge the gap between a patient's fear and their ability to remain still for the duration of the scan.

Despite the success of the three patients in this case series, the evidence remains fragmented. The PMC study was a small feasibility trial and lacked a control group, meaning the results cannot yet be generalized to the broader patient population. There is often a disconnect between a patient reporting a reduction in phobia on a questionnaire and their actual physical ability to tolerate the MRI environment.

Next Steps for VR Integration

While the metaverse-based approach provides a promising proof-of-concept, larger controlled trials are necessary to prove clinical efficacy. Future research must determine if VR training is significantly more effective than simple orientation or traditional sedation. Until then, the medical community is watching to see if these virtual simulations can move from small-scale trials to a standard part of cardiac patient preparation.
